Resumen:
In a clínica/ of rare frequency, firearms had inflicteda traumatic wound in thoracic duct.The presence of kilo microns in pleural drainageliquid, confirmed diagnosis.lnitial treatment was conservative, but it was decidedto resort to surgery, taking into consideration clinica/and paraclinica/ evidence of ma/nutrition, as we/1as alterations of the milieu intérieur. We identifiedoutpour of clear liquid from mediastine and so/ved itthrough a stitch on mediastinal ce// tissue, coveringthe site of leakage.On day 24 of evolution, sepsis appeares, the startingpoint being of bilateral pneumonitis. This conditionwas favorable resolved.
Se analiza un caso clínico de una entidad infrecuente,la herida traumática de conducto torácico producidapor arma de fuego. El diagnóstico se confirma porla presencia de quilomicrones en el líquido de drenajepleural. Inicialmente se realiza tratamiento conservador,pero ante la evidencia clínica y paraclínica dedesnutrición al igual que alteraciones en el mediointerno se decide la cirugía, identificando la salida delíquido claro del mediastino, resoviéndose medianteun punto sobre el tejido celular mediastinal incluyendoel sitio de fuga.Al día 24 de la evolución instala una sepsis a punto departida de múltiples focos de neumonitis bilateral, lacual se resuelve favorablemente y es dado de alta enbuenas condiciones.
