Calibration of the liverwort confluence in the left leads by neoplastic biliary obstructions
Calibrado de la confluencia de los hepáticos en las derivaciones izquierdas por obstrucciones biliares neoplásicas
Resumen:
A new surgical procedure is presented for the palliative treatment of pedicular malignant biliary obstruction: intubation of the hepatic ducts confluence in the left bypass operations by a right trans-hepatic tube, either a simple one or an U-tube. Its aim is to preserve drainage of both hemi-livers when the progression of the tumour infiltrates the aforesaid confluence. Maintenance of its viability avoids biliary stasis and severe angiocholitis in the contralateral undrained lobe, which shortens and complicates survival. Four cases were thus treated: three anastomosis in the left hepatic duct and one in the duct of segment 111. The post-operative courses were uneventful and average survivals of over 10 months were obtained, without relapse of jaundice or bouts of angiocholitis.
Se presenta un nuevo recurso técnico para el tratamiento paliativo de las obstrucciones neoplásicas pediculares: el calibrado de la confluencia de los hepáticos en las derivaciones izquierdas por un tubo transhepático derecho, simple o en seclal. Ello tiene por fina.lida-cl la preservación del drenaje de ambos hemihígados, cuando la progresión del tumor infiltra dicho confluente. Conservando la permeabilidad de ia convergencia, se evita la estasis biliar y angiocolitis grave del lóbulo contralateral no drenado, factor que acorta y complica la sobrevida. Cuatro casos fueron tratados por este procedimiento: tres anastomosis en el hepático izquierdo y una en el canal del segmento III. No hubieron complicaciones postoperatorias y se obtuvieron sobrevidas promedio de más de10 meses sin reaparición de fotericia y sin empujes de angiocolitis.
