Gastric ulcer in the distant evolution of the duodenal ulcer treated by gastroenterostomy
Úlcera gástrica en la evolución alejada de la úlcera duodenal tratada por gastroenterostomía
Resumen:
Two patients with duodenal ulcers and piloric obtructive syndrome had unrler gone gastroenterostomy far from the pilorus ( 28 and 32 years earlier). Theylater developed stomach ulcers complicated by haemorrhages. They were cured through degastroenterostomy and subtotal gastrectomy 75 % Billroth II. Thedifferent pathogenical hypothesis of gastric ulcers are discussed. Hyperacydity of antral gastrinic origin is considered the genetical ele.ment of gastric ulcers; this thesis is supported by the two clinical cases mentioned.
Se con1unican dos observ,;¡ciones de pacientes portadores de úlcera duodenal y síndrome obstructivo pilórico a los cuales se le practicó gastroenterostomía lejos del píloro (28 y 32 años antes) y que luego desarrollaron úlcera de estómago complicada con hemorragia. Se obtuvo la cur>.ción con la degastroenterostomíay gastrectomía su!)total 75 % Billroth II. A propósito de estas 0bservaciones se hacen algunos comentarios sobre las distintas hipótesis patogénicasde la úlcera gástrica. Se insiste en el factor hiperacides de origen gástrico antral como elemento genético de la úlcera gástrica, presentándose en favor de esta tesis los dos casosclínicos antedichos.
