Inmunoterapias personalizadas para el tratamiento del cáncer en Uruguay: Revisión narrativa

El cáncer posee una gran prevalencia y morbimortalidad mundialmente, siendo la segunda causa de muerte en Uruguay. Los tratamientos convencionales más utilizados actualmente incluyen cirugía, radioterapia y quimioterapia, teniendo estos grandes limitaciones como su baja selectividad, alta probabilidad de recidiva e importantes efectos adversos. De aquí la importancia de la investigación y desarrollo de nuevas terapias, dentro de ellas, las inmunoterapias, más recientemente las inmunoterapias personalizadas. Estas últimas actúan sobre dianas específicas del individuo para inducir la eliminación de las células cancerígenas por su propio sistema inmune. Este trabajo es una revisión narrativa, que pretende indagar sobre la aplicabilidad y viabilidad de las inmunoterapias personalizadas en Uruguay. Para ello, se definieron criterios de búsqueda en las principales bases de datos médicos con utilización de criterios booleanos y se creó una base de datos propia que contiene bibliografía relevante para su posterior análisis. Se comenzó por una revisión del estado de arte de las inmunoterapias. Posteriormente se seleccionó un tipo de inmunoterapia personalizada, la terapia con células CAR-T. Se profundizó sobre su producción, ventajas y desventajas, para la reflexión de su aplicabilidad terapéutica en nuestro país. Se concluye que la implementación de terapias CAR-T en Uruguay presenta un gran potencial para tratar neoplasias hematológicas, especialmente considerando la infraestructura y capacitación en curso. A futuro, el desarrollo de estrategias para superar las limitaciones en tumores sólidos podría posicionar a Uruguay como referente regional en inmunoterapia oncológica. Sin embargo, es fundamental analizar su viabilidad económica y no descuidar el énfasis en la prevención y diagnóstico precoz.

Cancer is a highly prevalent and deadly disease on a national and global scale, being the second most common cause of death in Uruguay. Conventional treatments of cancer include surgery, radiotherapy and chemotherapy. These treatments have important side effects, great recurrence rate, and low selectivity. Here lies the importance of investigating new therapies, highlighting immunotherapies, and within them personalized immunotherapies, as great innovations. These personalized immunotherapies target specific molecules to induce cancerous cell death mediated by the patient’s own immune system. This review intends to research applicability and viability of personalized immunotherapies in Uruguay. We begin with a review of the state of the art of immunotherapy in the treatment of cancer, and then move on to dig deeper into a personalized immunotherapy: CAR-T cell therapy. We proceed to delve into CAR-T’s production, advantages and disadvantages to then reflect on the applicability of this particular immunotherapy in Uruguay. In conclusion, the implementation of CAR-T therapies in Uruguay shows great potential for treating hematological neoplasms, especially considering the current infrastructure and ongoing training. In the future, the development of strategies to overcome the limitations in solid tumors could position Uruguay as a regional leader in oncological immunotherapy. However, it is essential to assess its economic feasibility and not overlook the emphasis on prevention and early diagnosis.
