La Neumonía Aguda Comunitaria es una infección respiratoria frecuente con elevada morbimortalidad. La prescripción indiscriminada de antimicrobianos representa un problema de salud pública debido al desarrollo de microorganismos resistentes. En este contexto surgen los Programas de Optimización de Antimicrobianos, como estrategias para su uso racional. El Hospital de Clínicas en el año 2018, creó un programa institucional que implementa guías de tratamiento de las enfermedades infecciosas más prevalentes. El objetivo de este trabajo fue medir la adherencia de los prescriptores a las pautas de tratamiento de la neumonía aguda comunitaria en el Hospital de Clínicas durante el año 2022. Se realizó un estudio descriptivo, transversal y retrospectivo mediante la revisión de historias clínicas electrónicas de pacientes ingresados en el período enero-diciembre 2022, con diagnóstico de neumonía al egreso hospitalario. Se incluyeron 130 pacientes. Se registró el score de severidad en el 6% de los casos, al 79% de los pacientes se les indicó un plan antimicrobiano acorde a las guías según grupo de riesgo y en los pacientes a los que se les realizó estudios microbiológicos, un 86% no mostraron hallazgos de relevancia diagnóstica. En relación a estudios previos se observó un incremento en el nivel de adherencia a las guías de tratamiento, en particular para el fármaco prescrito; un bajo registro del score de gravedad y/o grupo de severidad en la historia clínica y muy baja adherencia al switch a la vía oral. Este trabajo evidencia la necesidad de auditorías periódicas en relación a pautas institucionales de tratamiento.

Acute Community Pneumonia is one of the most frequent infections, with high morbidity and mortality and a common reason for prescribing antimicrobials. The indiscriminate use of these molecules represents a public health problem due to the development of resistant microorganisms that generate their inappropriate use. In this context, Antimicrobial Optimization Programs emerge as strategies for their rational use. In 2018, the Hospital de Clínicas created an institutional program that implements treatment guidelines for the most prevalent infectious diseases. The objective of this work was to measure the adherence of prescribers to the treatment guidelines for Acute Community Pneumonia at the Hospital de Clínicas during the year 2022. A descriptive, cross-sectional and retrospective study was carried out by reviewing the electronic medical records of patients admitted in the period January-December 2022, with a diagnosis of pneumonia at hospital discharge. 130 patients were included. It is highlighted that a severity score record was found in 6% of the cases, 79% of the patients were prescribed an antimicrobial plan according to the guideline’s recommendations based on the risk group and in patients who underwent microbiological studies, 86% did not show findings of diagnostic relevance. In relation to previous studies, an increase in the level of adherence to treatment guidelines was observed, particularly for the prescribed drug, maintaining a low proportion of the registration of the severity score or severity group in the clinical history, as well as a very low switch to oral route. This work shows the need for periodic audits in relation to institutional treatment guidelines.
