The immune system is an extensive and complex network made up of different cell types and soluble molecules, which give rise to cellular and humoral immunity. Both work together to guarantee the optimal functioning of this system. Regarding cellular immunity, there are CD4+ T lymphocytes that differentiate into different subpopulations, including Th17 cells that express RORγT and various cytokines such as IL-17. These cells, of heterogeneous nature, participate in tissue homeostasis and have been involved in the genesis of various pathologies. With the advancement of technology, attempts have been made to understand their plasticity and behavior in the different microenvironments within the organism, as well as the cellular differentiation processes that result in pro-inflammatory or regulatory phenotypes. In this review, the role of Th17 lymphocytes will be analyzed, and their plasticity and duality will be discussed in terms of their role in cancer, as well as in autoimmunity, specifically in multiple sclerosis. To achieve this, a bibliographic review will be carried out based on an exhaustive search of scientific research articles, through the use of virtual libraries, as well as other databases. It is of utmost importance to understand the dual role of Th17 lymphocytes in various pathologies in order to reflect on possible therapeutic opportunities and strategies for the treatment of both cancer and multiple sclerosis.
