Valoración de la técnica inhalatoria en usuarios del Hospital Maciel, año 2019. Evaluación de la técnica en pacientes con enfermedades bronco-obstructivas y en personal de salud.

Resumen:
Las enfermedades respiratorias figuran entre las causas de muerte más comunes a nivel mundial, siendo la patología bronco-obstructiva la de mayor relevancia y prevalencia. El eje del proyecto se centrará en su tratamiento actual poniendo énfasis en las terapias inhalatorias. Objetivos: Los objetivos principales radican en los errores más frecuentes que presentan los usuarios al enfrentarse con esta terapéutica y qué impacto tiene una adecuada educación previa en la técnica inhalatoria. Metodología: El diseño del estudio epidemiológico será de tipo longitudinal, prospectivo y de intervención. Se realizaría en el Hospital Maciel, Montevideo, Uruguay a usuarios de policlínicas de neumología, asma y EPOC. Se recolectarán datos de 40 pacientes, usuarios regulares de inhaladores con patologías respiratorias bronco-obstructivas. La metodología consiste en la observación de forma individual de la técnica de los pacientes seleccionados. Posteriormente se realizará un taller de carácter informativo utilizando materiales audiovisuales y por último evaluar nuevamente su técnica al uso del inhalador mediante una lista de chequeo. Las variables de mayor relevancia que se tendrán en cuenta son: la técnica correcta, educación/instrucción previa por parte del personal de salud a los usuarios, nivel de instrucción de los participantes y el tiempo de uso de inhaladores. Resultados: De los pacientes que no recibieron educación previa, ninguno logró ejecutar correctamente la técnica inhalatoria. Los que sí recibieron educación previa, solamente 29,03% ejecutaron la técnica adecuadamente. Del 77,5% de pacientes que realizaba la técnica inhalatoria de manera incorrecta, un 38,75% siguieron cometiendo errores post intervención. En cambio, un 61,2% revirtieron los errores logrando una técnica adecuada. No se evidencian diferencias significativas al examinar la técnica inhalatoria según nivel de instrucción de los participantes. De los pacientes que utilizaban el inhalador por más de 5 años un 50% cometió errores, mientras que los que lo utilizaban entre 1 y 5 años o menos de un año la mayoría cometieron errores. Conclusión: No se pudo evaluar si la educación previa recibida por los pacientes fue de calidad, si se puede concluir que una educación de calidad como la realizada en el taller impacta positivamente en la realización adecuada de la técnica inhalatoria, por lo que es importante realizar seguimiento a los pacientes y evaluar la misma cuando haya oportunidad.
