Intervenciones para promover el diagnóstico y tratamiento de sífilis en parejas sexuales de gestantes positivas. Revisión de alcance

En Uruguay se ha observado un aumento en la incidencia de sífilis congénita en el período 2020 - 2021, a pesar de que el diagnóstico y tratamiento está disponible para personas gestantes y sus parejas sexuales. Este estudio tuvo como objetivo identificar y analizar intervenciones para promover el diagnóstico y tratamiento de sífilis en parejas sexuales de gestantes positivas. Se realizó una búsqueda bibliográfica de artículos publicados entre 1994 y 2024 en las plataformas Pubmed, Cochrane, y Biblioteca Virtual en Salud. Se utilizó el software Rayyan para la selección y revisión de los estudios, mientras que el riesgo de sesgo fue analizado por los instrumentos de Joanna Briggs Institute para cada tipo de diseño. Se identificaron 11 estudios que cumplieron con los criterios de inclusión. La información fue organizada en tablas de extracción de datos con las categorías de interés, y se realizó un análisis y síntesis cualitativa de dichos datos. Los resultados mostraron que la notificación por parte de la gestante generó una mayor predisposición de la pareja sexual para acercarse al servicio de salud y recibir tratamiento. Además, se vieron estrategias para la notificación como la implementación de mensajes de texto y llamadas telefónicas. Entre los hallazgos principales, ofrecer apoyo e información sobre la enfermedad por parte del sistema de salud hacia la embarazada fue uno de los métodos más efectivos. Como estrategia para aumentar el tratamiento y diagnóstico de la pareja se evidenció que la implementación de pruebas rápidas fueron efectivas, ya que permitían el tratamiento conjunto del binomio pareja-gestante en una misma instancia. Esto llevó a la conclusión de que ofrecer asesoramiento y otras estrategias de notificación así como el empoderamiento de las gestantes, son los métodos más efectivos para aumentar el diagnóstico y tratamiento de las parejas sexuales de las gestantes positivas.

In Uruguay, the incidence of Congenital Syphilis has increased during the years 2020-2021 despite its diagnosis and treatment being available for all pregnant women and their sexual partners. To achieve this, a scoping review was conducted including studies published between 1994 and 2024 found in Pubmed, Cochrane, and “Biblioteca Virtual en Salud”. The platform Rayyan was used to select and revise the studies. The risk of bias was analyzed using the critical appraisal tools developed by the Joanna Briggs Institute for each study design. 11 studies that met the eligibility criteria were included. From which, a data charting table was created using categories of interest, to summarize and systematize the information of the studies. An analysis and a qualitative synthesis of the information was performed. Given that the implications of the disease are highly unknown and that there is fear of violent reactions among women when disclosing their diagnosis to their sexual partners, one of the most effective methods was to educate and support pregnant women. Additionally, it was found that when women informed their sexual partners about their diagnosis, it led to a greater willingness from the sexual partners to attend the health care facility and therefore receive treatment. It was also found that the use of rapid tests was effective in tackling this problem, because women received the test results much faster, enabling the treatment of pregnant women and their sexual partners at the same time. Moreover, several barriers to diagnosis and treatment were identified, such as fear of violence triggered by the disclosure of the diagnosis, limited clinic hours, denial of the diagnosis, and lack of awareness regarding the detrimental effects of the disease on the fetus. This led to the conclusion that offering advice and additional notification strategies along with support and complete information about the disease, is one of the best strategies to increase the diagnosis and treatment of the sexual partners of pregnant women who test positive for syphilis.
