Knowledge about carcinogenesis and metastasis development has had an exponential growth, generating great advances in relation to treatments, trying to reach for individualized therapies rather than traditional general ones. The immune system plays a key role in terms of the aspects underlying neoplastic development, this is the fact that made this system a target of investigation, in order to develop therapies that enhancing its functions could have an impact in tumor growth (immunotherapy). Consequently, the objective of these treatments is to obtain an efficient immune response towards malignant cells that would normally be inhibited by the neoplasm. Furthermore, within these evasion mechanisms, the PD1 receptor is in charge of programmed cell death, and its interaction with the PDL1 ligand, which attenuates the normal immune response, is to be distinguished. One of the most innovative strategies is Pembrolizumab, a monoclonal IgG4 antibody that has a negative effect in the inhibition caused by PD1´s interaction, as was mentioned before. This therapy is approved in Uruguay for the treatment of non-small cell lung cancer and stage IV melanoma, being financed by the Fondo Nacional de Recursos (FNR). Although these are targeted therapies and have no comparison in terms of adverse effects to chemotherapy, they are not exempt from risks. Within them are found the immune-related adverse events (irAEs), that include diseases with autoimmune background produced by the blockage of immunological regulation. They can be particularly severe, causing great morbidity, and up to the point of needing the suspension of treatment. This study intends to investigate the frequency of immune-related adverse events of the previously mentioned drug, in the Uruguayan population financed by the FNR, between 2018 and 2022. Its relevance lies in the fact that, up to this date, there is not enough analysis of real-life data of the use of this drug in Uruguay, nor in the nearby region. This would be the first report of immune-related adverse events in our country.
