Resumen:

El presente estudio describe el impacto de las técnicas de diálisis lenta extendida asociada a hemodiafiltración con reposición en línea (SLED-HDF-OL) y hemodiálisis intermitente (HDI) en la hemodinamia sistémica y regional cerebral de pacientes críticos. Se trata de un estudio piloto de tipo observacional, descriptivo y prospectivo, enmarcado dentro de un proyecto financiado por el Fondo de Investigación en Nefrología, representando el punto de partida para evaluar el impacto de la técnica en pacientes críticos. Materiales y métodos: se seleccionaron dos pacientes adultos con insuficiencia renal, daño multiorgánico, bajo soporte vasopresor, con indicación de SLED-HDF-OL y/o HDI, ingresados en la Unidad de Cuidados Intensivos del Hospital de Clínicas “Dr. Manuel Quintela”. Para evaluar la hemodinamia regional cerebral se midió la saturación cerebral regional de oxígeno (rSO2) mediante espectroscopía cercana al infrarrojo, y para evaluar la hemodinamia sistémica se midió la presión arterial sistólica (PAS), la presión arterial diastólica (PAD) y la presión arterial media (PAM), mediante un catéter intraarterial. Métodos de análisis: las variables fueron adquiridas por el sistema CONTINE y procesadas mediante el software (“Procmx”) el cual brindó los gráficos e índices (TOxa) para elaborar conclusiones. Resultados: en ambos pacientes se observaron variaciones en la presión arterial (PA), rSO2, teniendo ambos una autorregulación cerebral (AC) deficiente, independientemente del tipo de diálisis y el valor de PAM. Conclusiones: la recomendación de presión óptima intradiálisis no toma en consideración la individualidad de cada paciente. Utilizar las estrategias de medición aplicadas en este proyecto permitirán individualizar el manejo de la presión arterial óptima en función del estado de autorregulación de la AC en pacientes en diálisis, independientemente de la técnica utilizada.

It is a pilot observational, descriptive, and prospective study situated within a project funded by the Nephrology Research Fund, working as an initial step for assessing and understanding the impact of this technique on a larger cohort of patients. Materials and Methods: The selection process involved two adult patients with severe acute kidney injury, multiorgan failure, undergoing inotropic support, and indication of SLED-HDL-OF and/or HDI, admitted to the Intensive Care Unit of “Dr. Manuel Quintela” Hospital. Regional cerebral hemodynamics were assessed by measuring regional cerebral oxygen saturation (rSO2) using near-infrared spectroscopy (NIRS). The patient’s systemic hemodynamics were evaluated by measuring systolic blood pressure, diastolic blood pressure, and mean arterial pressure using an intra-arterial catheter. Methods of Analysis: the variables recorded by the CONTINE system were processed using the “Procmx” software, which delivered graphical outputs and indices (TOxa) used to draw conclusions. Results: both patients exhibited variations in SAP, DAP, and rSO2, with both demonstrating impaired cerebral autoregulation (CA), regardless of the dialysis technique used and the mean arterial pressure. Conclusions: the recommendation of optimal intradialysis blood pressure does not account for patient individuality. The measurement strategies used in this project can work to enable the individualization of optimal blood pressure management based on the state of cerebral circulation autoregulation (CA) in patients undergoing dialysis, regardless of the technique employed.
