Cruces moleculares entre Diabetes Mellitus tipo 2 y Enfermedad de Alzheimer apoyados en la bioinformática estructural

Resumen:

Diabetes Mellitus tipo 2 (DM2) y Enfermedad de Alzheimer (EA) son enfermedades crónicas prevalentes, especialmente en población añosa, y en los últimos años su relación ha sido objeto de estudio. Uruguay cuenta con población envejecida, por tanto estas entidades ocupan relevancia en los servicios de salud. Se han generado hipótesis sobre la vinculación en sus bases moleculares y su esclarecimiento permitiría conectarlas a nivel diagnóstico y terapéutico, pudiendo ocasionalmente prevenirlas y/o tratarlas mediante un mismo plan terapéutico. Metodología: Se exploró la bibliografía del campo, identificando los protagonistas proteicos: insulina en DM2, Aβ y tau en EA, y sus cruces moleculares a través de IDE, RI, RAGE y AGEs, buscando y analizando estructuras que los evidencian. Las herramientas usadas de bioinformática estructural de proteínas permitieron obtener y analizar: a) secuencias proteicas de estos sistemas extraídos de Uniprot, b) sus estructuras 3D localizadas en PDB y c) las interacciones insulina/AGEs para DM2 y Aβ para EA en posibles sitios de unión con IDE, RI y RAGE actuando como estructuras comunicantes entre ambas patologías. El análisis se completó con visualización molecular bi/tridimensional usando Discovery Studio Visualizer 2024. Resultados: Tenemos tres ejes principales: a) los sistemas moleculares que participan en DM2, describen formas de agregación de la insulina, RI y la IDE, b) los sistemas moleculares en EA, con énfasis en Aβ 1-40 y 1-42 y la proteína tau, c) el cruce entre DM y EA con RAGE. Conclusiones: La literatura consultada plantea una relación entre DM2 y el riesgo de desarrollar EA. Los sistemas seleccionados y sus interacciones revelan mecanismos compartidos como la resistencia a la insulina, neuroinflamación y estrés oxidativo, conectando ambas enfermedades a nivel molecular.

Type 2 Diabetes Mellitus (T2DM) and Alzheimer Disease (AD) are two very common chronic diseases, particularly in the elderly and their relationship has been investigated in recent years. These conditions are exceedingly relevant in Uruguay, a country with an aging population, and they receive great attention through the health services. At a molecular level, hypotheses have suggested a link between these syndromes, and understanding this connection would enable diagnostics and therapies to be developed to allow for the prevention or treatment of both conditions using a single treatment plan. Metodology: we explored the literature in the field, identifying the key proteins involved in both pathologies (insulin in T2DM, Aβ and tau in AD), and their molecular intersections through the IDE, RI and RAGE and AGEs, looking for and analyzing structures that highlight these interactions. Using structural bioinformatics tools, we examined: a) the protein sequences from UniProt, b) the 3D structures available in PDB, and c) the interactions between insulin/AGEs in T2DM and Aβ in AD, focusing on potential binding sites with IDE, RI, and RAGE. These molecular interactions could act as "bridges" linking the two diseases. To further explore these connections, we also used Discovery Studio Visualizer 2024 to visualize the structures in both 2D and 3D Results: Based on three main axes: a) focusing on the molecular systems involved in type 2 diabetes , describing the aggregation forms of insulin, IR and IDE; b) molecular systems in Alzheimer's disease, with emphasis on Aβ 1-40 and 1-42 and tau protein; c) the intersection between DM2 and AD with RAGE. Conclusions: The reviewed literature suggests a relationship between DM2 and the risk of developing AD. The selected systems and their interactions reveal shared mechanisms such as insulin resistance, neuroinflammation, and oxidative stress, linking both diseases at the molecular level.
