Estrategia de estudio de anticuerpos antifosfolipídicos en pacientes asistidos en Hospital Pasteur

Introducción: El Síndrome Antifosfolipídico (SAF) es una entidad clínica poco frecuente, con complejidad en su diagnóstico y manejo terapéutico. Nos proponemos realizar un estudio descriptivo y analítico sobre características clínicas, epidemiológicas y estrategia diagnóstica de anticuerpos antifosfolipídicos en pacientes asistidos en el Hospital Pasteur en los últimos 12 meses. Metodología: Estudio descriptivo retrospectivo. Se realizará un muestreo por conveniencia, se incluirán pacientes que se les solicite anticuerpos antifosfolipídicos en Hospital Pasteur durante un año. Resultados: el 55% presentaron eventos clínicos con criterios clasificatorios donde el 29% corresponde a eventos venosos, el 19% a arteriales y el 7% a obstétricos. Un 33% presentó condiciones clínicas asociadas a Anticuerpos antifosfolipídicos (AAF) y un 26% no presentó eventos que justificaran su solicitud. Otras condiciones que motivaron la solicitud fueron EAS un 9% y Glomerulopatías 12%. La proporción de solicitud adecuada en evento venoso fue significativamente mayor que en evento arterial (75% vs 12.5%, p=0.0198). Se constató SAF en un solo paciente, que presentó un perfil de triple positividad de los AAF y se confirmó a las 12 semanas. Discusión: La enfermedad tromboembólica venosa fue la manifestación más frecuentemente constatada, lo cual coincide con estudios previos que muestran que el evento clasificador más frecuente al debut es venoso. Los eventos arteriales se dieron en menor proporción, siendo el accidente cerebro vascular la presentación más frecuente. Considerando la media de edad y la alta prevalencia de FRCV en este subgrupo, la sobre indicación de AAF es un aspecto que merece ser destacado en pacientes con eventos arteriales. En relación a la solicitud adecuada de AAF, fue significativamente mayor en eventos venosos en relación a los arteriales. La estrategia de estudio de AAF sigue siendo subóptima, constatándose los pacientes con eventos arteriales un subgrupo de especial interés para optimizar los protocolos de solicitud de AAF.
