Resumen:

Introducción: Los recién nacidos pretérminos del Uruguay constituyen el 9%,2 por lo que surge la necesidad de buscar estrategias adecuadas para su atención. Entre estas estrategias se destacan las medidas mínimamente invasivas para lograr apoyar la transición a la vida extrauterina, teniendo como objetivo disminuir la morbimortalidad de esta población. En este sentido, la maternidad del hospital de clínicas ha desarrollado estrategias en las que se incluye la administración de surfactante por vía orofaríngea. Objetivos: Evaluar y comparar los resultados neonatales en pacientes en los que se les administró surfactante por vía orofaríngeo al nacer versus pacientes a los que no se les administró surfactante a través de esta vía. Metodología: Estudio observacional de Cohorte-Retrospectivo. Se dividieron en 2 grupos, un grupo control de 18 pretérminos menores de 32 semanas de edad gestacional nacidos en el periodo de 2014-2016, los cuales no recibieron surfactante por vía orofaríngea al nacer y un grupo de casos de 28 pretérminos, nacidos en el periodo de Julio 2017- Agosto 2019 que recibieron surfactante orofaríngeo. Resultados: La técnica se logró aplicar de forma reproducible y segura. Se observó una disminución en la necesidad de AVM, días de CPAP y días de Oxigeno respecto al grupo de casos, siendo esta diferencia no significativa. Respecto a las dosis de surfactante endotraqueal en el grupo de casos se observó una disminución en la mediana de dosis requeridas respecto al grupo control, siendo esta diferencia no significativa. Conclusión: La administración de surfactante por vía orofaríngea al nacer puede ser considerada una práctica efectiva, factible, segura y simple, aplicable en centros no especializados en la recepción de recién nacidos pretérminos. Con este protocolo, es posible disminuir maniobras invasivas en los recién nacidos pretérminos. Este trabajo aporta evidencia significativa para el desarrollo de un estudio controlado de mayor magnitud.

Results: This technique was applied safely and is reproducible. Regarding the results, there was a decrease in the need of AVM, CPAP days and oxygen therapy days in the case study group, even though this difference was not significant. Regarding the doses of endotracheal surfactant in the case study group, a decrease in the median dose required was observed, in comparison to the control group, this difference was also not significant. Conclusion: These results demonstrate that the administration of surfactant by oropharyngeal route can be considered an effective, feasible, safe and simple option that can be applied in centres not specialized in the reception of preterm infants. This protocol proved that it can be possible to reduce invasive techniques in preterm newborns. This study provides significant evidence for the development of future larger controlled studies.
