Resumen:

Introducción: La fluidoterapia es un pilar terapéutico en el manejo inicial del paciente gran quemado, pero debe evitarse la hiper-reposición en la resucitación de estos pacientes porque puede asociarse a complicaciones. Nos proponemos evaluar los factores asociados a la sobrehidratación y su impacto en la morbimortalidad. Metodología: Estudio retrospectivo observacional, en el que se incluyeron todos los pacientes grandes quemados ingresados al CENAQUE entre Enero 2019 y Mayo 2024. Se registraron características demográficas, lesionales e índices de severidad de la injuria, ventilación mecánica invasiva (VMI) al momento del ingreso al CENAQUE y diagnóstico de Injuria Inhalatoria (II). Estas variables fueron contrastadas entre el grupo de pacientes con balance hídrico acumulado significativo (BHAS) y aquellos con balance no significativo (BHANS). El punto de corte de BHAS se estableció en 17 litros mediante regresión logística binaria para mortalidad ajustada por SCTQ y peso al ingreso. Se realizó análisis multivariado para establecer aquellos factores asociados a BHAS. Resultados: 125 pacientes fueron incluidos, los del grupo BHAS (55 pacientes) tuvieron mayor %SCTQ (p<0,001), score de ABSI (p= 0,001) y de BAUXr (p<0,001), se encontraron ventilados con más frecuencia al ingreso (p = 0,015) y a las 48hs (p <0,001). La dosis acumulada de Morfina (p 0,003) y midazolam (p0,001) al 2° día fue mayor y requirieron más días de ARM (p= 0,002). No se encontraron diferencias significativas para la edad, peso al ingreso, II y estadía en CENAQUE. El análisis multivariado mostró asociación de BHAS con una mayor SCTQ y VMI día 2. El grupo BHAS presentó una mayor frecuencia de complicaciones incluyendo insuficiencia respiratoria, insuficiencia renal aguda (IRA), necesidad de terapia de reemplazo renal (TRR) y síndrome compartimental de miembros. Conclusión: La SCTQ y la presencia de VMI se asociaron a BHAS. Los pacientes con BHAS presentaron mayor frecuencia de complicaciones.
