Microbiota y patología respiratoria: revisión sobre los conocimientos más recientes

Hasta hace poco tiempo se creía en los pulmones como un ambiente estéril, tan solo se conocía la colonización de las vías aéreas superiores. Gracias a los avances técnicos en métodos de cultivo y secuenciación se ha logrado identificar la microbiota pulmonar. Estos microorganismos, junto a la ya conocida microbiota de las vías aéreas superiores, conforman toda la microbiota respiratoria, la cual está compuesta por una diversidad de bacterias, virus y hongos. La composición de la microbiota varía por factores externos (edad, desarrollo, población, tabaquismo, etc.), así como también por factores internos. Distintas condiciones de pH, temperatura, PO2, PCO2 que se dan a lo largo del tracto respiratorio determinan distintos nichos anatómicos. Estos nichos se ven afectados por una variedad de patologías frecuentes del aparato respiratorio, desde infecciones del sistema respiratorio o patologías inflamatorias, hasta patologías pulmonares malignas. La microbiota normal de estos nichos anatómicos puede verse alterada, en un proceso conocido como disbiosis, lo cual podría llevar a procesos de inflamación e inmunomodulación, que resultan partícipes en la patogenia, la progresión y también en la resolución de estas enfermedades. El conocimiento de estos procesos va cobrando cada vez más importancia por su aplicación en la clínica, específicamente como elementos diagnósticos, pronósticos e incluso terapéuticos. En esta revisión se realiza una puesta a punto de los avances en el área, específicamente sobre el rol y vinculación de la microbiota respiratoria con el sistema inmune, las infecciones respiratorias, la patología respiratoria crónica y el cáncer de pulmón.

Until recently, the lungs were believed to be a sterile environment, and only the colonization of the upper airways was known. Thanks to technical advances in sequencing methods, the lung microbiota has been identified. These microorganisms, together with the upper airway microbiota, make up the entire respiratory microbiota, which is made up of a variety of bacteria, viruses and fungi. The composition of the microbiota varies by external factors (age, development, population, smoking, etc.), as well as by internal factors. Different conditions of pH, temperature, PO2, PCO2 that occur throughout the respiratory tract determine different anatomical niches. These niches are affected by a variety of common pathologies of the respiratory system, such as infections, inflammatory pathologies or malignant pulmonary pathologies. The normal microbiota of these anatomical niches can be altered, in a process known as dysbiosis, which could lead to inflammation and immunomodulation processes, which are involved in the pathogenesis, progression and also in the resolution of these diseases. The knowledge of these processes is gaining more and more importance due to its clinical application, specifically as diagnostic, prognostic and even as therapeutic elements. This review is made to resume the advances in the area, specifically on the role and linkage of the respiratory microbiota with the immune system, respiratory infections, chronic respiratory pathology and lung cancer.
